1) Brief description of your full-time work experience. What do you do?
5.5 years of work ex. started as a Quality Engineer in a Software services firm, and later moved to Tech. Consulting firm as a Senior Quality Analyst. Promoted and changed gears about an year and half back as a Business Analyst . My daily job includes interacting with clients of N.America & Canada and providing them solutions in the space of Enterprise Project Mgmt (Managing budget, material & non-material resources, reporting).
2) Your GMAT - Unofficial score - 700 Q48, V37; Did pretty well on AWA and IR (Expecting >=5 and >6 respectively)
Fourth attempt, so pretty much the last.
3) College info: Not among the top 20 Engineering colleges in India, scored a meagre 68%; highest in class was 82%. 2007 passout. Had pretty bad health during 3 year of grad. tried as much as I can recover the percentage in 4th year with 79%. Math and Stats scores >75 throughout.
4) Significant college and post-college extra-curricular activities or community service, especially leadership experience.
a. Co-editor of a monthly newsletter, which caters to around 800+ people, at my firm
b. Lead CSR activities, at my firm, during 2010 - 2011 (Had to give up after transitioning as a BA because of frequent domestic travel and work load). Planning for annual CSR day, year-long awareness programs and stuff.
c. Volunteer at one of the local NGOs in my city -- mainly teaching kids of 8-12 years during weekends
d. Core committee member of Computer Society of India during college. Organizing bi-annual events and inviting corporates to college for industry exposure
5) Important certifications like CFA, CPA, FSA, or CA.
None
6) Your target programs.
I'm looking at following programs and would like to know the universities which are in my reach
US: Tepper, CMU; McCombs, Univ. of Texas; Kenan-Flager, UNC; and Mays, TAMU.
Singapore: NUS and Nanyang
UK: Sad, Oxford and Judge, Cambridge
India: ISB (though I believe it's a stretch!)
7) When you plan to apply? Please specify round and application cycle, for example R1 2013-14 or R3 2012-13.

8) Your post-MBA goal.
Short term: To get into Consulting industry -- specifically those firms which have strong foothold in community and social services , or into General Management.
Long term: (Read as looong term goal!) Set up a for-profit or non-for-profit firm targeted at children's education and community well-being (still vague to put it in words)
