When to apply what all to be kept in mind.

Hello,

I am aiming for admission in fall 2013 (Jul-Aug 2014) and planning to attempt GMAT during early September this year.
Request some guidance here, though I got huge help from various blogs but would be very helpful to iron out a few doubts.
1. If I attempt the GMAT on say 10th Sep this year and an application deadline is on 1st Oct, would there be enough time to apply for the college with the scores?
PS:Since I stay in India, it will take a while for the application hardcopy (+recommendations+essays) to reach the college through courier.
2. By when should I have the the undergrad transcripts, recommendations, essay ready assuming 1st Oct deadline?
3. Does the GMAC sends the official scores to colleges that we select right before attempting the GMAT?
4. To my knowledge the content of application package should consist of filled application form, recommendations (if not sent directly), essay, undergrad transcripts; is there anything missing in the list?
